Treatment/Previous Treatment: add missing questions, fix yes/no and NA, show comments only when relevant

Users have reported several issues in the Treatment/Previous Treatment section: the "evidence, previous treatment, yes, no" field is incorrectly reading "NA" and needs to show "yes" or "no"; the comments section is pulling in text when the answer is "no," which is unnecessary and should be changed; and all NA readings in this section need to be fixed. The critical question "Has a company treated this structure" and its yes/no checkbox is missing and should appear ahead of the current field. The question "Has a company treated at the time of inspection" is also missing and needs to be included with yes/no actively showing; related fields can be NA or left off if blank, but the required state questions must be present.

Why it matters: Treatment history is critical for WDO reports; missing required questions and incorrect yes/no or NA display undermines compliance and clarity.
