Quote Acceptance Action not Triggering

We have had some issues in some of our instances the quote acceptance actions not triggering. Here are three examples Michelle sent me today. I looked at the action flows in each of these quotes and all the accepted actions are in pending status still.

- RIA - (TEST)306 Clearbrooke Way
- OHI - (TEST)5830 Lullaby Ln

- NHI -(TEST)411 Waters Edge Ct

I double checked the conditions on these actions and they are set up the same way as the other brands that are working
